Ranking Physical Therapy Treatments (from Worthless to Elite)

All physical therapy treatments are not created equal. Here’s your evidence-based guide to ranking physical therapy treatments – so you know what actually works, and what’s a waste of time.

Level 1: Novice

A lot of old school treatments fall into “placebo purgatory.” They don’t do much to help tissues heal. They might feel good for a few minutes. But that’s about it.

Level 1 treatments include therapeutic ultrasound and e-stim (electrical stimulation). Neither treatment does much to heal injured tissue.

Likewise, ice and heat may offer temporary relief, but they have limited benefits. And despite the hype, the truth about ice baths isn’t as glamorous as advertised.

I won’t dissuade someone from using these remedies at home if they find them effective. But the best PTs rarely use these interventions in the clinic.

Level 2: Intermediate

These interventions work better than placebo for treating orthopedic issues. A little better.

These passive treatments relieve pain and improve function. But they aren’t particularly powerful on their own.

Elite physical therapists use these treatments. Just not in isolation.

Level 3: Advanced

Highly skilled physios cook up exercise-heavy treatment plans.

These PTs rely on pain-relieving movements like McKenzie exercises, general aerobic and strength exercise, and specific mobility exercises to target stiff regions.

In short, excellent PT treatment plans lean heavily on exercise.

Level 4: Elite

Finally, true experts combine Level 2 and Level 3 treatments (manual therapy + exercise), while considering patient preferences and their own expertise. 

Exercise + hands-on treatment produces better outcomes than either approach alone. 

Elite physios also identify and address relevant lifestyle factors that influence healing – like stress, sleep, physical activity, and nutrition.

And finally, excellent PTs consider a client’s biology, psychology and social context when tailoring a custom treatment plan to meet their needs.

  1. I disagree. I have needed physical therapy for injuries multiple times over the last 50 years .Ultrasound ,e stim,massage and manual therapy have relieved pain and swelling so that I could then engage in exercise. I recently dealt with cervical radiculopathy,the pain was excruciating. In three treatments,which you say are worthless,I was pain free and able to progress to exercise.
